色素性乾皮症（XP）C群について
先日、色素性乾皮症（XP）C群と診断された海外の患者様よりお問い合わせをいただきました。今回は、C群という病型について、そしてXP全体のタイプ別の特徴について、わかりやすくまとめてご紹介します。
We recently received an inquiry from a customer overseas who has been diagnosed with Xeroderma Pigmentosum (XP) Complementation Group C. In this article, we would like to explain what Group C is, as well as the characteristics of each type of XP.
色素性乾皮症のC群は、徹底した遮光（紫外線対策）によって皮膚がんの発症や多発を防ぐことができれば、進行性の神経症状を伴うことなく、良好な生命予後が期待できるタイプです。
Complementation Group C of XP is a type in which a favorable life expectancy, without progressive neurological symptoms, can be expected if strict UV protection prevents the onset and recurrence of skin cancer.
XP患者様の生命予後を大きく左右する要因のひとつに「進行性の神経症状」がありますが、C群は基本的にこの神経症状を伴わないタイプに分類されます。そのため、紫外線を完全に遮断し、皮膚がんの発症・多発を防ぎきることができれば、一般の方とほぼ変わらない寿命を全うすることも可能とされています。
One of the major factors affecting the life expectancy of XP patients is “progressive neurological symptoms.” Group C is generally classified as a type without these neurological symptoms. Therefore, if UV exposure can be completely blocked and the onset or recurrence of skin cancer prevented, it is considered possible to live a lifespan close to that of the general population.
ただし、注意すべき点があります。C群は日焼けによる赤みや水ぶくれといった初期の「光線過敏症状」が目立ちにくい傾向があるため、病気そのものの発見が遅れやすいという特徴があります。気づかないうちに紫外線ダメージが蓄積し、10代〜20代という若い年代から皮膚がんが多発してしまうリスクが非常に高いのです。
However, there is an important point to note. Group C tends to show less noticeable early “photosensitivity symptoms,” such as redness or blistering from sunburn, which often leads to delayed diagnosis. As a result, UV damage can accumulate unnoticed, creating a very high risk of multiple skin cancers appearing as early as the teenage or early adult years.
つまり「UVにさえ当たらなければ長生きできる」という言葉の前提には、幼児期からの極めて厳格で、生涯にわたる遮光管理が不可欠であるということを、私たちは強くお伝えしたいと思います。
In other words, we want to emphasize that the premise behind “long life is possible if UV exposure is avoided” requires extremely strict, lifelong UV management starting from early childhood.
色素性乾皮症は、原因遺伝子の違いによってA〜G群の7つの相補性群と、V（バリアント）型を合わせた計8つのタイプに分類されます。大きく分けると、わずかな紫外線でも激しい日焼けを起こす「サンバーン増強型」と、日焼けは目立たないもののシミやがんが多発する「色素異常型」の2つの傾向に分かれます。
Depending on the responsible gene, Xeroderma Pigmentosum is classified into eight types in total: seven complementation groups (A through G) plus the Variant (V) type. Broadly speaking, these fall into two tendencies: a “severe sunburn” tendency, in which even minimal UV exposure causes intense sunburn, and a “pigmentary abnormality” tendency, in which sunburn is not very noticeable but pigment spots and cancers occur frequently.
